- Periodic evaluation & oral cancer screening
- 2 hygiene visits (cleanings) per year
- Regular dental x-rays as needed
- 2 fluoride treatments every 12 months
- 1 emergency exam
- Up to 20% off additional treatment
Periodontal Plan
$799 / year
- Periodic evaluation & oral cancer screening
- 4 periodontal maintenance treatments
- Regular dental x-rays as needed
- 2 fluoride treatments every 12 months
- 1 emergency exam
- Up to 20% off additional treatment
Youth Plan (12 & under)
$390 / year
- 2 check-up exams per year
- 2 pediatric hygiene visits per year
